Addressing chronic diseases often requires new strategies to delay health deterioration, improve function, and address the problems that people confront in their day-to-day lives.

That’s why Virginia Arrhythmia Consultants has established a Remote Patient Monitoring Program (RPMP) that is designed to empower individuals with chronic diseases to better manage their care and improve their quality of life, through the assistance of one or more monitoring devices, and a care team working under the supervision of your physician.

This low-cost or free program is focused on helping you share your daily measurements with your care team so that they can help you improve your overall health status.

Eligibility

Remote Patient Monitoring services are a Medicare and (most) private insurance covered benefit for individuals with one or more chronic conditions that your physician feels could benefit from frequent monitoring.

Convenient Access

For the convenience of patients, this program is primarily carried out over the phone or through electronic communications, eliminating the need for our patients to make a special visit to participate.

How the RPMP Works

You will be provided with a simple to use cellular monitoring devices such as a blood pressure cuff, scale, glucometer (for measuring blood glucose), or pulse oximeter (for measuring blood oxygen).

The device will AUTOMATICALLY and IMMEDIATELY transmit your measurements to your care team. You do not need a cellular phone, an “app”, or an internet connection. You will also have access to a portal to see all of your measurements and ongoing trends.

You and your care team will communicate monthly by phone, video chat, and/or email and text about your readings to ensure your health is stable, and potential strategies and approaches if you are interested in improving your readings.
